Per Diem - Direct Care

Per Diem - Direct Care - $15.25/Hr
Closing Date: 01/02/2023

Per Diem employees fill-in for regular status employees who may not be available. Most of the need is on second and third shift, and weekends. This is a great opportunity for a college student or anyone that wants to make a difference. Fill-in staff needed to support program participants in our mental health programs on an as-needed basis.
 
First/Second Shift. Responsibilities include: teaching skills such as cooking, menu planning, laundry, grocery shopping and providing support for ADL skill development. Documentation of services in the Electronic Medical Records System (EMRS). CNA/HHA/PCA/PCT preferred.
 
Third Shift. Responsibilities include: light housekeeping, regular bed checks, monitoring mental and physical health of members, and medication cueing when necessary. Documentation of services in the Electronic Medical Records System (EMRS). CNA/HHA/PCA/PCT preferred.
 
Hours: Varied to include evenings, overnights, and weekends.
 
Duties and Responsibilities:
  • Assists program participants in recreational, social, vocational, residential, and community events
  • Assists members in performing daily living skills and meeting daily living needs such as shopping, laundry, housekeeping, transportation, etc.
  • Assists with the implementation of goal activities as defined on the individual’s service plan
  • Provides crisis intervention supports as needed
  • Provides emotional support to program participants
  • Monitors and documents program participant’s physical and mental health and report relevant information to the appropriate staff
  • Provides conditions that maximize safety for staff and program participants
  • Provides coverage as needed at any of the Agency sites per Agency need
  • Provides coverage as needed at any of the Agency sites per Agency need
  • Engages in continuous concerted efforts to access, obtain, respond to pertinent agency information/communications, and disseminates as appropriate
  • Provides working conditions that maximize safety for staff and program participants
  • Maintains CPR/FA Certification
  • Monitors and documents interactions and observations
  • Other job-related duties as assigned
  • Performs duties while demonstrating Agency Core Competencies and values

 
Education and Experience:
·        High School Diploma or equivalent
·        Work experience in the provision of mental health services preferred
·        Valid CT Driver’s License, vehicle registration and insurance
·        Good driving record
